The data collection tool used in this study is the observation sheet, using checklist using SOP (Standart Operating Procedures) of Siloam Hospitals and secondary data taken from the medical record. The method used in this research is quantitative study of documentation study with sampling using purposive sampling with total 35 medical record. The purpose of this research is to know the description of the assessment of pain numeric rating scale by the nurses in the inpatient ward of 1 Siloam Hospitals Bali. In the treatment of pain, nurses play an important role in assessing, providing appropriate interventions, and documenting. Pain is the most frequent reason in seeking health care assistance so that the need for proper management of pain by health workers, especially nurses. Data from the Word Health Organization / WHO (2015) indicates the prevalence of acute and chronic pain is increasing every year, should be the world's attention, as well as the Indonesian government. Pain is an unpleasant sensory and emotional experience resulting from actual or potential tissue damage, or is described as damage itself.
